What is Midline Diastema? 

It refers to Anterior midline spacing between the two maxillary central incisiors. 


CAUSES/ ETIOLOGY-


1) MIDLINE PATHOLOGY-

Midline diastema may occur because of of some soft tissue and hard tissue  pathologies such as cyst, tumor and odontomes . 


2) IATROGENIC-

Midline diastema occurs when rapid maxillary expansion is undertaken. 

3) TOOTH MATERIAL ARCH- LENGTH DISCREPANCY-

Conditions such as missing teeth, microdontia and extraction. 



4) TRANSIENT MALOCCLUSION-

As seen in ugly duckling stage. 


5) PRESSURE HABITS-

Such as thumb sucking, tongue thrusting predispose to midline diastema. 


6) ABNORMAL FRENAL ATTACHMENT-

Presence of thick and fleshy labial frenum. 



7) UNERUPTED MESIODENS-


Presence of an unerupted mesiodens between the two central incisiors predispose to midline diastema.



DIAGNOSIS-


1) BLANCH TEST-


It is performed to diagnose a fleshy labial frenum. 


2) TOOTH - MATERIAL - ARCH LENGTH DISCREPANCIES-

Determined  using model analysis. 



TREATMENT-


IT IS DONE IN 3 PHASES

1) REMOVAL OF CAUSE. 

2) ACTIVE  TREATMENT

3) RETENTION


1) REMOVAL OF CAUSE-

a) Etiology should be removed.


b) Habits should be eliminated.

c) Unerupted mesiodens should be extracted.

d) FRENECTOMY-  To excise a thick fleshy frenum.

e) Midline pathology should be treated.



2) ACTIVE TREATMENT-

A)  REMOVABLE APPLIANCES TO TREAT                   MIDLINE DIASTEMA-
1)  FINGER SPRING

2) SPLIT LABIAL BOW





B) FIXED APPLIANCES TO TREAT MIDLINE
      DIASTEMA-


1) M SHAPED SPRINGS

2) ELASTIC THREAD OR ELASTIC CHAIN

3) CLOSED COIL SPRING. 






3) RETENTION-

Retention is obtained through suitable retainers.

1) Lingual bonded retainers.

2) Banded retainers.

3) Hawley's retainers.




Role of cosmetic retainers-

Composite resins are used to close midline DIASTEMA in adult patients.
